Abstract:The health care sector in South Africa is in need of major reform. Change is necessary to accommodate a new environment and should be directed at two fundamental objectives: 1) The improvement of access by the underserved. 2) The curbing of the rapidly escalating costs of health care. This paper evaluates the options available to South Africa to meet these objectives. Greater emphasis can be placed on market mechanisms relying on private provision of care and private insurance to finance those services. Alternatively, greater reliance can be placed on the public sector either to provide services directly or to finance the use of private services. The paper examines the pros and cons of privatization, government policy in this respect, prospects for the private sector and for a National Health Service, efficiency of a National Health Service, equity in the financing and distribution of health services, and the health care system and AIDS.
