| Abstract: | A growing body of evidence illustrates that South Africa's health indicators compare unfavourably with other countries of similar GNP and are on a par with a number of developing countries in sub-Saharan Africa. There is, however, a wide degree of differentiation in the distribution of diseases and in health care services within South Africa, between homeland areas and within the homelands themselves. A description of expenditure on health care, infant mortality rates, and health care infrastructure in Ciskei and Transkei shows a comparative superiority of primary health care provision in Ciskei. In order to show how health care is received at the village level, two case studies are examined: the first from a village in Matatiele, Transkei, based on research carried out in the period 1982-1984, and the second from a Ciskeian village in Keiskammahoek, based on research commenced in 1990. Given the imminent reincorporation of the homelands into South Africa, models of regional decentralization for health care should be investigated. Regional decentralization would appear to be a way of maintaining the gains made in some areas whilst upgrading services in others. Bibliogr., notes, ref. |
